Job Description
Position background

• Network Coordinator will be responsible requesting credentialing documents, tracking provider onboarding process, receiving and submitting LA Care Attestation.



Please describe the team the candidate will be working with?

• Candidate will be working with the Director of Program Management and providers. skill sets team culture etc.

• Customer Service, Excel, Uploading documents into a STFP site.



Responsibilities:

• Reach out to providers to obtain required documents to credential providers along with a copy of the LA Care Provider Orientation Handbook and Training along with the Attestation form.

• Obtain the completed copies of the required credentialing documents and Attestation forms back from provider.

• Upload into the required documents into database and submit to cred.

• Once credentialed send provider to the next step depends on the CDO provider is being added.



Required skills/attributes:

• Computer skills, Excel uploading to SFTP sites, customer services. Making out bound calls when necessary.
